- Please make sure all sections are completed. Incomplete applications will not be processed.
- The applicant confirms all information provided in this application is true to the best of applicant’s knowledge.
- While we would like to help all deserving individuals and families, we run on limited funds.
- The Foundation reserves the right to approve or reject an application and the Foundation will not provide any explanation for its decision.
- You confirm any assistance you would receive from the Foundation for Local Needy will be reported to IRS on your tax return.
On confidentiality and background checks. The Foundation tries its best to keep the applicant’s information and documents confidential. The Foundation at the same time has an obligation to make sure the funds are distributed to the deserving individuals and families. By applying to Foundation you grant The Foundation for Local Needy the right to do financial, criminal and other background checks on you as deemed necessary.
